Hyundai HL757-9 Cummins QSB 6.7

I've got a Hyundai HL757-9 with a Cummins qsb6.7 24 anyone know what would cause all 4 valves on #6 cylinder to break?

    This guy talks about it, but only mentions the exhaust valves.
