Hard to say without more information. Does your cap look like the one in the picture below? If so the lever needs to be held up at the end the padlock goes in. You will have to hold up on the lever while turning to get the other end to drop into the notch in the actual cap. If machine is being used in very dirty conditions and has not had the cap off for some time the notch in the actual cap could be filled with dried mud or what ever and it might take some washing or raping with a small hammer to clean it out. Worst case if you still can't get the lever to engage the notch you can carefully cut away the outer cap housing with a cut-off wheel. Go around the outer cap below where the #13 is pointing. Once you have that off you can then remove the actual cap. Only problem is you will not have the locking feature. Here is another view of the cap, the red part is just a cover that could if need be cut off just above the snap-ring(#30) shown at the bottom. The red part is a separate item that can be ordered part number 5D1911 and most dealers should have them or get them in a day or two for about $22.00

If it has the vandalism cap as shown it is fairly easy to get cross threaded. Hold the tang up and turn with a crescent wrench. If you don't need the vandalism cap take it off and use the stock cap you will have better luck.

It screws off. Just hold up on the little lever and start unscrewing the cap. As kshansen said, the lever should fall into a notch. When it does just keep unscrewing it while holding up on the lever.

In couple of rare cases I have had to pound a screw driver in the slot where the tab is hinged to the cap.

Maybe this will help. The "actual cap" is the part colored GREEN in this picture. And yes sometimes if they have been on for awhile they will need a bit of persuasion to get them to start turning. You could also remove the pin that the lever pivots on and remove the lever to let you jam something down in the notch in the "actual cap" then use an adjustable wrench or pair of pliers to turn it.
